MD Cosmedical Solutions only collects information which is necessary for one or more of its business functions. This information is obtained directly from you, such as when you voluntarily provide it through the ‘Contact Us’ form on our website, when you visit our boutique in person, or over the phone. ° If you engage us to facilitate a procedure or treatment for you, or enquire about your eligibility for a particular procedure or treatment, we will generally collect your contact information, such as your name, email address and phone number. In addition, we may also collect information which may be considered ‘Sensitive Information’ under the Privacy Act where necessary, such as your date of birth, photographs of you, consent to treatment, medical history and medicare number, to help us perform that procedure.
° If you engage us to facilitate a procedure or treatment for you, we will generally collect your payment details, such as your debit or credit card details, in order to fulfill payment of those services provided to you.
Sensitive Information
Sensitive information is defined in the Privacy Act to include information or opinion about an individual’s race, ethnicity, political views, membership in political groups, religious or philosophical beliefs, membership in trade unions or professional organisations, criminal records or health information.
Sensitive information will be used by us only:

° Where required or authorised by law.
Collection from Third Parties
Where reasonable and practicable to do so, we will collect your Personal Information only from you. However, in some circumstances we may be provided with information by third parties. In such a case we will take reasonable steps to ensure that you are made aware of the information provided to us by the third party.
Direct Marketing
Like most businesses, we use direct marketing to promote our business from time to time. We may add your email address to our email marketing database so we can send you information and updates about products and services we think you may be interested in. If we send you an email that does not relate to your personal cosmedical interests, you will have the option of unsubscribing from receiving further communications of that nature.
